Pastoral Reference Form — Action Required

Before you proceed, you need to download the Pastoral Reference Form. This form must be completed and signed by your pastor or religious leader. You will be required to upload the filled-in form in one of the upcoming steps.

Here's how it works:
  1. 1Download the Pastoral Reference Form using the button below and give it to your pastor to fill in.
  2. 2Fill in your personal details on this page (Section A) and submit to receive your login credentials via email.
  3. 3Log in to the Applicant Portal to complete Sections B & C (qualifications, program selection, and document uploads).
  4. 4Upload the completed Pastoral Reference Form along with your other supporting documents when prompted.
Important: Please download the form now so your pastor has enough time to complete it before you reach the document upload step.
